    Dear Valued Customer,

    We are sending this letter to notify you that we are required by Florida Tax Law Section 212.05 to collect sales tax from our customers on the sale or rental of tangible personal property and specifically enumerated software related services unless our customers provide us with a valid and fully completed Florida sales and use tax exemption or resale certificate.

    If your business is exempt from Florida sales and use tax by statute, nature of your business, or because you intend to resell, please submit a valid and fully completed Florida sales and use tax exemption or resale certificate to us so that we may adjust and/or verify your Florida sales and use tax exempt status in our billing system.

    It is important to promptly send a valid and fully completed Florida sales and use tax exemption or resale certificate form to maintain your correct Florida sales and use tax status in our billing system. If we do not receive a valid and fully completed Florida sales and use tax exemption or resale certificate within 30 days from the date of this letter, we will begin charging sales tax on the applicable products and services and will invoice you for any applicable Florida sales tax on your account(s).

    If you have any questions on how to properly and fully complete your applicable exemption or resale certificate form, please review the instructions for each applicable form listed on the following Florida State Department of Revenue website:

    Important Information on Fully Completing and Submitting Your Sales/Use Tax Exemption or Resale Certificate

    A list of generally acceptable Florida sales and use tax exemption or resale certificate forms can be found below:

    • DR-13 – FL Annual Resale Certificate for Sales Tax
    • DR-14 – Consumer Certificate of Exemption
    • DR-97 – Suggested Format for Blanket Exemption Certificate Based on Property's Use

    You may use one State and Local sales and use tax exemption or resale certificate for multiple tax exempt accounts, as long as you include each customer account number on the face of the sales and use tax exemption certificate or as a separate attachment submitted back to us along with the requested sales and use tax exemption certificate. The State or Local sales and use tax exemption or resale certificates may be evaluated by KPMG LLP (a public accounting firm we have engaged to assist us with the handling of the exemption/resale certificate process, including obtaining new or updated exemption certificates from our customers) for completeness.

    What If We Have Already Submitted Sales/Use Tax Exemption or Resale Certificate in the Past?

    You may have already submitted a valid State or Local sales and use tax exemption or resale certificate to us in the past. However, we are requesting that you fully complete a current sales and use tax exemption or resale certificate(s) at this time so that we can update or confirm your correct State and Local sales tax status in our billing system.

    How to Submit Your Valid and Fully Completed Sales and Use Tax Exemption or Resale Certificate

    Submit your valid and fully completed State and Local sales and use tax exemption or resale certificate to a ticket within your account.

    Fax
